You progress through three distinct levels — a yellow office maze, a dark industrial zone, and a flooded poolrooms labyrinth — by scavenging for items, solving environmental puzzles, avoiding hostile entities, and managing your stamina and mental stability. Each run is seeded differently, so room locations and codes change every time.

Equip the Flashlight in your hand and right-click to toggle it on or off. The Backrooms are dark — keep it handy, but be aware that light may attract attention.
You have a stamina bar. Sprinting drains it. When stamina reaches zero, you are forced to stop sprinting and briefly slowed (exhaustion). Stamina regenerates while walking or standing still. Stamina Pills fully restore it for 30 seconds. Conserve your sprinting for emergencies.
Mental stability drains slowly over time and faster when hostile entities are nearby. If it bottoms out, the Paranoia effect intensifies. Use Mental Stability Pills to recover. Stability only matters while inside the Backrooms — it resets when you leave.
Step into any oak 1×2 locker to hide. While hiding, you are not detected by Bacteria or other threats. Leave the locker when it is safe to continue.
Brief objective hints and important item notifications appear on the bossbar at the top of the screen. They stay visible for about three seconds before fading. Keep an eye on them when you pick up new items or reach key areas.
All key items are found by looting containers scattered through the Backrooms levels. There are no crafting recipes for Backrooms items — everything must be scavenged in-world.
Supply Bags hold up to 18 slots of random loot including food (bread, apples), Mental Stability Pills, Stamina Pills, a Baseball Bat (rare), and occasionally a Room Compass. Office Safes hold 27 slots and have better odds for pills and the Baseball Bat. Supply Crates also hold 27 slots and are guaranteed to contain at least one of the three critical Level 2 objective items (Industrial Battery, Main Fuse, Maintenance Keycard).
The Red Room Compass and Blue Room Compass point toward their respective special rooms in Level 1. These rooms contain unique rewards and clues — the Blue Room houses the CRT Monitor terminal.
Find and repair the broken CRT Monitor in the Blue Room. Once repaired, it displays a persistent 4-digit code. This code is unique to your current run and is needed to progress. Interact with the terminal to read the code (it appears as a named paper item labeled CRT ACCESS CODE).
The mod is structured across three levels, accessed in order.
A finite 400×400 yellow-wallpaper office labyrinth. Your goal is to gather three items — the Industrial Battery, Main Fuse, and Maintenance Keycard — and locate the freight elevator. Along the way:
Once all three objective items are in your possession, use them at the elevator to power it and ride it to Level 2.
A larger, darker industrial area. Bacteria roam the space and emit positional ambient sounds — listen carefully to judge their distance and direction. The Industrial Battery, Main Fuse, and Maintenance Keycard are guaranteed to spawn here (spread apart), so even if you missed them in Level 1, you can find them now. Repair the freight elevator to proceed to Level 3.
A 40×40 flooded maze with a poolrooms aesthetic (pale tiles, submerged corridors, eerie ambient sounds). You enter through a slide entrance that drops you into the water. Your objective is to solve the 5-color slide-lock puzzle:
